What should I prepare for my initial consultation?
After the initial FREE Connection Call, you will be sent a client agreement, in-take form, comprehensive questionnaire and 5-day food journal to be completed prior to the initial consultation. It is important that these forms are submitted in a timely manner so the Health Coach can start your personalized health and wellness plan! You can also submit any copies of your previous lab work for the Health Coach to review. Come with your excitement and willingness to start your journey to a healthier, more balanced life!
How much does it cost?

Do you bill insurance for these services?
Unfortunately, most insurance companies do not cover the type of services that PIVOT Integrative Consulting, LLC provides, so the cost is out of pocket. The personalized health and wellness coaching from PIVOT Integrative Consulting, LLC, combined with regular visits with your Primary Care Provider, provide significant benefits to your personal health and wellness!
